How to Apply

Home Owners
To be eligible:- Your house, apartment, unit etc. must have been built before 1978.
- The owner/occupant(s) must qualify under the Income Guidelines.
- There must be at least one child under the age of 6 years of age either living in or regularly visiting (over 6 hours per week, 60+ hours per year) the home, or a pregnant woman living in the household.
- You must complete the application by printing out the pdf from the link below. The application is pre-fillable. You will need to include copies of all of the items applicable to you on the “Income Checklist” (i.e., bank statements, tax return, etc. on page 3).
- Incomplete applications will not be considered.
- Anyone listed on the deed must sign the application and provide identification (e.g., Driver’s license or state issued ID).

Rental Property Owners or Tenants
To be eligible:- Your rental property must have been built before 1978.
- The current (and/or future occupants must qualify under the Income Guidelines.
- There must be at least one child under the age of 6 years of age either living in or regularly visiting (over 6 hours per week, 60+ hours per year) the home, or a pregnant woman living in the rental unit.
- You must complete the application by printing out the pdf form from the link below. The application is pre-fillable. You will need to include copies of items applicable to you on the “Income Checklist” (i.e., bank statements, tax return, etc. on page 3).
- Incomplete applications will not be considered

- Property Owners of rental units will need to fill out the “Landlord Information” section on the bottom of the page 1 of the application. Tenants will need to complete the top portion of page 1 entitled “Occupant Information” and pages 2-3 of the application in their entirety. Both occupants and landlords must sign and date on the bottom of page 3 of the application and provide identification (e.g., Driver’s license or state issued ID).

Once a completed application is received, the Lead Intake Clerk/ Interviewer will contact you to schedule an appointment where you can bring in copies of your documents.
